The National Healthcare Safety Network (NHSN) definitions are critical for standardizing healthcare-associated infection surveillance in US healthcare facilities. However, their use in accurately detecting healthcare-associated transmission (HAT) has not been measured. Using whole-genome sequencing surveillance data, we show that the NHSN has a sensitivity of 44.4% in detecting HAT.

Background:
Whole-genome sequencing (WGS) has traditionally been used in infection prevention to confirm or refute the presence of an outbreak after it has occurred. Due to decreasing costs of WGS, an increasing number of institutions have been utilizing WGS-based surveillance. Additionally, machine learning or statistical modeling to supplement infection prevention practice have also been used. We systematically reviewed the use of WGS surveillance and machine learning to detect and investigate outbreaks in healthcare settings.
Methods:We performed a PubMed search using separate terms for WGS surveillance and/or machine-learning technologies for infection prevention through March 15, 2021.
Results:Of 767 studies returned using the WGS search terms, 42 articles were included for review. Only 2 studies (4.8%) were performed in real time, and 39 (92.9%) studied only 1 pathogen. Nearly all studies (n = 41, 97.6%) found genetic relatedness between some isolates collected. Across all studies, 525 outbreaks were detected among 2,837 related isolates (average, 5.4 isolates per outbreak). Also, 35 studies (83.3%) only utilized geotemporal clustering to identify outbreak transmission routes. Of 21 studies identified using the machine-learning search terms, 4 were included for review. In each study, machine learning aided outbreak investigations by complementing methods to gather epidemiologic data and automating identification of transmission pathways.
Conclusions:WGS surveillance is an emerging method that can enhance outbreak detection. Machine learning has the potential to identify novel routes of pathogen transmission. Broader incorporation of WGS surveillance into infection prevention practice has the potential to transform the detection and control of healthcare outbreaks.

Background:
Identifying routes of transmission among hospitalized patients during a healthcare-associated outbreak can be tedious, particularly among patients with complex hospital stays and multiple exposures. Data mining of the electronic health record (EHR) has the potential to rapidly identify common exposures among patients suspected of being part of an outbreak.
Methods:We retrospectively analyzed 9 hospital outbreaks that occurred during 2011–2016 and that had previously been characterized both according to transmission route and by molecular characterization of the bacterial isolates. We determined (1) the ability of data mining of the EHR to identify the correct route of transmission, (2) how early the correct route was identified during the timeline of the outbreak, and (3) how many cases in the outbreaks could have been prevented had the system been running in real time.
Results:Correct routes were identified for all outbreaks at the second patient, except for one outbreak involving >1 transmission route that was detected at the eighth patient. Up to 40 or 34 infections (78% or 66% of possible preventable infections, respectively) could have been prevented if data mining had been implemented in real time, assuming the initiation of an effective intervention within 7 or 14 days of identification of the transmission route, respectively.
Conclusions:Data mining of the EHR was accurate for identifying routes of transmission among patients who were part of the outbreak. Prospective validation of this approach using routine whole-genome sequencing and data mining of the EHR for both outbreak detection and route attribution is ongoing.
Asian snake farms: conservation curse or sustainable enterprise?

Snake farming in Asia has increased over the past decade, and conservationists have expressed concerns that farms may foster overexploitation of wild populations and create legal conduits for illegally harvested wild individuals. We conducted face-to-face interviews with snake farmers in Viet Nam and China, with the aim of describing the basic models under which snakes are farmed for meat. We synthesized this information to assess the feasibility of farming snakes for human consumption, drawing conclusions about the impact of this industry on the conservation of wild snake populations. The most commonly farmed snakes include the monocled cobra Naja kaouthia, the Chinese cobra Naja atra, the oriental rat snake Ptyas mucosus and the king cobra Ophiophagus hannah. These species have life histories that are compatible with the demands of intensive livestock production, including early maturity, rapid growth rates, high reproductive output, efficient food assimilation rates and undemanding space requirements. Snake farmers appear to be capitalizing on the unique energy-efficiency of snakes to produce meat for human consumption. We conclude that the ease and profitability of farming snakes in China and Viet Nam make farming a viable substitute for harvesting wild snakes, with apparently minimal threat to wild populations. Snake farming offers a range of novel agricultural opportunities and has the potential to play a pivotal role in sustainable development.

Cephalopods (Mollusca: Cephalopoda) play an important role as keystone invertebrates in various marine ecosystems, as well as being a valuable fisheries resource. At the World Malacological Congress, held 21–28 July 2013 in Ponta Delgada, Azores, Portugal, a number of cephalopod experts convened to honour the contribution of the late Malcolm R. Clarke, FRS (1930–2013) to cephalopod research. Endorsed by the Cephalopod International Advisory Council (CIAC), the meeting discussed some of the major challenges that cephalopod research will face in the future. These challenges were identified as follows: (1) to find new ways to ascertain the trophic role and food web links of cephalopods using hard tissues, stable isotopes and novel concepts in theoretical ecology; (2) to explore new approaches to the study of cephalopod morphology; (3) to further develop cephalopod aquaculture research; (4) to find new ways to ascertain cephalopod adaptation and response to environmental change; (5) to strengthen cephalopod genetics research; and (6) to develop new approaches for cephalopod fisheries and conservation. The present paper presents brief reviews on these topics, followed by a discussion of the general challenges that cephalopod research is bound to face in the near future. By contributing to initiatives both within CIAC and independent of CIAC, the principle aim of the paper is to stimulate future cephalopod research.

Antimicrobial Use in Outpatient Hemodialysis Units

Objective.
To quantify and characterize overall antimicrobial use, including appropriateness of indication, among patients receiving chronic hemodialysis.
Design.Retrospective and prospective observational study.
Setting.Two outpatient hemodialysis units.
Patients.All patients receiving chronic hemodialysis.
Methods.The rate of parenteral antimicrobial use (number of doses per 100 patient-months) was calculated retrospectively from September 2008 through July 2011. Indication and appropriateness of antimicrobial doses were characterized prospectively from August 2010 through July 2011. Inappropriate administration was defined as occasions when criteria for infection based on national guidelines were not met, failure to choose a more narrow-spectrum antimicrobial on the basis of culture data, or occasions when indications for surgical prophylaxis were not met.
Results.Over the 35-month retrospective study period, the rate of parenteral antimicrobial use was 32.9 doses per 100 patient-months. Vancomycin was the most commonly prescribed antimicrobial, followed by cefazolin and third- or fourth-generation cephalosporins. Over the 12-month prospective study, 1,003 antimicrobial doses were prescribed. Among the 926 (92.3%) doses for which an indication for administration was available, 276 (29.8%) were classified as inappropriate. Of these, a total of 146 (52.9%) did not meet criteria for infection, 74 (26.8%) represented failure to choose a more narrow-spectrum antimicrobial, and 56 (20.3%) did not meet criteria for surgical prophylaxis. The most common inappropriately prescribed antimicrobials were vancomycin and third- or fourth- generation cephalosporins.
Conclusions.Parenteral antimicrobial use was extensive, and as much as one-third was categorized as inappropriate. The findings of this study provide novel information toward minimizing inappropriate antimicrobial use.
Engineered Nanostructured Coatings for Enhanced Protein Adsorption and Cell Growth

We designed and produced pure cubic zirconia (ZrO2) ceramic1 coatings by an ion beam assisted deposition (IBAD) with nanostructures comparable to the size of proteins. Our ceramic coatings exhibit high hardness and a zero contact angle with serum. In contrast to hydroxyapatite (HA), nano-engineered zirconia films possess excellent adhesion to all orthopaedic materials. Cell adhesion and proliferation experiments were performed with a bona fide mesenchymal stromal cell line (OMA-AD). Our experimental results indicate that the nano-engineered cubic zirconia is superior in supporting growth, adhesion, and proliferation. Since cell attachment is mediated by adhesive proteins such as fibronectin (FN), to elucidate why cells attach more effectively to our nanostructures, we performed a comparative analysis of adsorption energies of FN fragment using quantum mechanical calculations and Monte Carlo (MC) simulation both on smooth and nanostructured surfaces. We have found that a FN fragment adsorbs significantly stronger on the nanostructured surface than on the smooth surface2.

Teouma, an archaeological site on Efate Island, Vanuatu, features the earliest cemetery yet discovered of the colonizers of Remote Oceania, from the late second millennium B.C. In order to investigate potential migration of seventeen human individuals, we measured isotopes of strontium (87Sr/86Sr), oxygen (δ18O), and carbon (δ13C), as well as barium (Ba) and strontium (Sr) concentrations, in tooth enamel from skeletons excavated in the first two field seasons. The majority of individuals cluster with similar isotope and Ba/Sr ratios, consistent with a diet of marine resources supplemented with plants grown on the local basaltic soils. Four outliers, with distinctive 87Sr/86Sr and δ18O, are probably immigrants, three of which were buried in a distinctive position (supine, with the head to the south) with higher Ba/Sr and δ13C, consistent with a terrestrial, nonlocal diet. Among the probable immigrants was a male buried with the crania of three of the locally raised individuals on his chest.

The Argentine shortfin squid (Illex argentinus) is a common neritic species occurring in waters off Brazil, Uruguay, Argentina, and the Falkland/Malvinas Islands in the southwest Atlantic. Illex argentinus is the most important fished cephalopod species in the area and plays a significant role in the ecosystem. It is object of major fisheries using both trawlers (mostly from European countries) and jigging vessels (mainly from Asian countries) and estimated total annual average catch for the last 15 years (1988-2003) is about 700 000 tons. The present paper aims to develop predictive models of squid abundance in relation to physical and environmental conditions, models that could ultimately be applied to fishery forecasting. Fishery and biological data collected by scientific observers aboard commercial trawlers between 1988 and 2003 were analysed in relation to physical and environmental factors to establish the spatio-temporal pattern of the species' distribution and quantify the influence of environmental variables (e.g. SST, depth) on local abundance. The data included 26 168 fishing haul records, of which 11 103 were positive for Illex. CPUE (Catch Per Unit Effort, kg h−1) was used as abundance index. The analyses were based on time-series maps created using Geographical Information Systems (GIS). GIS maps showed that highest CPUE values were recorded during the first four months of the year (the Austral summer-autumn), with peak values higher than 5000 kg h−1 mainly located within 42° S, 46° S and MN (North part of Malvinas/Falkland) areas. Generalised additive models (GAMs) were used to describe variation in Illex argentinus abundance in relation to geographical and environmental variables. The presence/absence (PA) of Illex and its abundance (CPUE) in areas of presence were modelled separately. Predictors retained in the optimal models included SST, latitude, longitude, month, average fishing depth and year. Both models suggest a clear seasonal effect: maximum catchability was found during March (PA model) and the maximum abundances were found during the first quarter of the year (CPUE model). GAM models also demonstrated that higher catches and maturity of squid were related, in general terms, to warmer and deeper water.
Infrared Cathodoluminescence Studies from Dislocations in Silicon in tem, a Fourier Transform Spectrometer for Cl in Tem and Els/cl Coincidence Measurements of Lifetimes in Semiconductors

In this paper we report the first observations by cathodoluminescence in TEM of radiative infrared emission from dislocations in silicon. These results were obtained at 23-25K in a Philips EM400 electron microscope. Our results are interpreted in the light of earlier photoluminescence work, and the prospects for obtaining spectra from a small number of well characterized dislocations with high spatial resolution are reviewed. Preliminary results are also reported showing the infrared band-gap emission spectrum of GaInAs at 90K recorded using a small Fourier Transform Spectrometer fitted to the same electron microscope. Finally, we describe timing coincidence experiments from samples in which only those visible CL photons are counted which arrive at the detector at the same time as the corresponding transmitted electron energy loss event. We describe the use of this technique for life-time mapping in semiconductors.
